As of Spring 2009, South Central Scholars (SCS) staff and board have revised the renewal application process to include a separate submission for overall progress and evaluation. Please fill out this form to the best of your knowledge. The information that you provide in this survey will be used to review current SCS program initiatives, strategies, and progress. In addition, the information used will update information on your file to reflect your current status, including employment status, field/interest, and other important information that will allow SCS staff, volunteers, and mentors to better serve you.
All courses and grades for this Progress Report should be for the 2009-2010 school year that you have completed. The courses and grades stated here will be compared to your transcript.
